Camper Waiver

WAIVER: All DAY, KIDS & TEEN Camps (including S.A.L.T., & OUTBOUND)

1. My child and I understand the nature and inherent hazards and risks of the intended activities related to participation in camp activities including but not limited to injury or death arising from: child's failure to follow instructions of supervisors; communicable illness; and independent acts of third parties not under the control of supervisors. We acknowledge that engaging in these activities may require a degree of skill and knowledge and that my child has responsibilities as a participant. In order to minimize risks to my child or other participants, I will take responsibility to see that my child is prepared for all activities and is in good health before his/her arrival. I understand that I am to inform the camp of any special circumstances that may affect my child's ability to fully participate in camp activities. This would include serious behavioural problems.
2. My child and I understand that Sunnybrae Bible Camp reserves the right to dismiss a camper due to disciplinary action or for reason of a communicable disease or lice detection.
3. My child and I authorize the camp health care providers to order x-rays, routine tests, and/or treatment, and to provide/arrange necessary related transportation for my child. In case of medical emergency, I understand that every reasonable attempt will be made to contact me, or the emergency contact named below. However, in the event that I or my named contact cannot be reached, I give my permission to the personnel in charge of Sunnybrae Bible Camp to secure emergency medical treatment for my child. I agree to pay for any charges for emergency medical treatment that are not covered by personal health insurance.
4. I understand that my child's photo may be taken for promotional purposes. I hereby authorize and consent to the use of images or videos of my child, with or without their name, by Sunnybrae Bible Camp, for purposes including but not limited to: promotional material, printed publications, internet posts including social media, television, and other media sources
5. I authorize the managing of medications that accompany my child and the managing of common over-the-counter medications that might be needed. See OTC Form for more information.
6. Sunnybrae Bible Camp, as part of the Fellowship Baptist Churches in the interior region of BC, may share my child's contact information with the Fellowship Baptist Church in our residential area for the purpose of follow-up after camp.
7. COVID-19 Addendum: Sunnybrae Bible Camp Communicable Disease Prevention Plan. My child and I agree to act in compliance with the Sunnybrae Bible Camp Communicable Disease Prevention Plan to reduce the spread of COVID-19. We understand the contagious nature of COVID and will not hold Sunnybrae Bible Camp liable for transmission.
8. I understand that by submitting this application I am affirming having legal custody of the child named.

I have read the above and by signing below:

• I agree it is my intention to have my child participate in various camp activities.
• I understand and fully accept the risks involved and I agree to assume those beyond the control of the Camp staff.
• I release Sunnybrae Bible Camp, its directors, and staff members from any liability in the event of illness, accident, or misfortune that may occur to my child during camp.
